Compensation
- Gross Weekly Pay: $2626.56
- Weekly Taxable Amount: $1000.0
- Weekly Stipend/Non Taxable Amount: $1626.56
You must be duplicating expenses to receive stipends. If you are driving versus relocating, we will instead offer mileage reimbursements. Rates assume Travel. Locals within 50 miles may have a rate adjustment.

Experience
- Minimum 2 years acute hospital case management experience required
- Previous travel experience required
License & Certification Required
- California state license preferred
- Pending license accepted
- MSW certification required
Skills Required
- Knowledge of drug/alcohol abuse treatment options
- Experience working with patients experiencing homelessness
- Experience with patients with mental health history or in crisis
- Biopsychosocial assessment
- Care coordination
- Discharge planning
- Documentation according to plan of care
- Documentation of patient/caregiver teaching/learning
- Provide patient and family education, support, and advocacy
- Ability to organize and participate in family meetings
- Participate in the development of the treatment plan
- Performing role of care coordination and discharge planning
- Computerized documentation experience required
- Charting system experience with Epic required
Desired Skills
- Experience in facilitating discharge and contingency planning
- Knowledge of cultural issues
- Experience with Pediatric patients
- Experience with Geriatric patients
- Experience with Adult patients

Requirements
- Current Resume with up to 7 year work history, with gaps explained - position, unit, job type, city, state must be indentified for each assignment
- Negative TB Test (Within 12 Months)
- Titers - MMR/Hep B/Varicella
- Current - Tdap/Flu Vaccinations
- Must be able to pass pre-employment, background and drug screening
