Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform case management duties with current acute hospital experience in the role within the last 6 months
• Utilize InterQual criteria for admission reviews and patient assessments
• Document patient information using Epic charting system
• Manage caseload of up to 20-22 patients with 1:20 patient ratios
• Coordinate care for adult and geriatric patient populations
• Conduct admission criteria assessments for emergency department patients
• Work with HMOs, IPAs, and other insurance entities
• Perform patient assessments, care coordination, and discharge planning
• Collaborate with interdisciplinary support including social services
• Complete needs assessment and order durable medical equipment (DME)
• Process prior authorizations for patient care
• Develop and implement patient plans of care
• Work at least 2 weekend shifts in a 4-week schedule to meet department needs
• Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Maintain valid California state nursing license
• Float between facilities within the state as required
• Complete required modules pre-start and annually (average 16 hours)
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Obtain LA fire card within the last 30 days
• Use facility-provided communication devices when required for assignment duties
• Self-report time spent on non-billable training modules
• Complete skills checklist as part of submission requirements
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
