Position Summary:
VitalHire is seeking a skilled and compassionate PTA - Physical Therapist Assistant Rehab to join our growing clinician community. Key Responsibilities:
• Provide physical therapy assistant services in home health setting • Maintain minimum of 30 points weekly for productivity • See average of 5-7 patients per day • Provide care for geriatric patients with various diagnoses including CHF, COPD, pneumonia, MS, ALS, diabetes, and post-surgical conditions • Work within up to 60-mile radius territory and maintain flexibility with territory assignments • Complete timely documentation during patient visits using HCHB charting system • Call physicians to request changes to Care Plan and obtain frequency/duration orders in collaboration with PT • Maintain current BLS certification and valid state licensure • Complete 40 hours of orientation and required training programs • Adhere to dress code requirements wearing scrubs of any color • Use facility-provided iPad for documentation and patient care activities • Maintain timely communication with supervisors and healthcare team • Work scheduled shifts (Tue-Sat or Mon-Fri) with weekend coverage requiring manager approval • Complete minimum of 35 patient visits per week • Obtain pre-approval from manager for any overtime or holiday work • Submit requested time off at time of application • Maintain active driver's license and personal transportation for patient visits • Complete required Hospice/Home Health Checklist • Collaborate with interdisciplinary team including physical therapy, interpretation services, and social services
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
