Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Burbank, CA. Key Responsibilities:
• Provide case management services for adult and geriatric patients
• Manage caseload of up to 25 patients
• Perform care coordination and collaborative care activities
• Develop comprehensive plans of care to meet patient and caregiver needs
• Conduct discharge planning and needs assessments
• Order durable medical equipment (DME) as needed
• Participate in care conference activities and family meetings
• Participate in the development of treatment plans
• Provide acute medical social work services including both social work and discharge planning roles
• Assist patients with drug/alcohol abuse treatment options
• Support patients experiencing homelessness
• Provide services for patients with mental health history or in crisis
• Handle advance directive and POLST processes
• Navigate protective services processes when needed
• Utilize Epic charting system for documentation
• Work one full weekend every 4 weeks
• Commit to working 3 of the 5 major holidays
• Float between all facilities within the state as required
• Complete required modules and training programs pre-start and annually
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete 40 hours of orientation
• Maintain required certifications (MSW or LCSW)
• Complete skills checklist and compliance documentation
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
