Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Santa Rosa, CA. Key Responsibilities:
• Provide case management services for adult and geriatric patients • Maintain current BLS certification and required licensure • Utilize Epic charting system for computerized documentation • Organize and participate in family meetings • Participate in the development of treatment plans • Determine medical necessity per evidence-based guidelines • Utilize InterQual criteria for patient assessments • Coordinate care with interdisciplinary team including IV teams, physical therapy, respiratory services, and social services • Perform discharge planning and coordinate post-acute care services • Arrange DME, SNF placement, and ambulance transport independently • Work with HMOs, IPAs, and insurance companies for prior authorizations • Complete needs assessments and order durable medical equipment • Perform data abstraction and maintain compliance with HIPAA guidelines • Ensure compliance with CMS regulations and Joint Commission standards • Complete required modules and training programs pre-start and annually • Work 3 out of 5 major holidays as required • Work two weekends every six weeks • Float between facilities within the state as needed • Maintain patient ratio of up to 20 patients • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty • Complete skills checklist and compliance documentation • Self-report time spent on non-billable training modules • Utilize facility-provided communication devices when required
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
