Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Santa Rosa, CA. Key Responsibilities:
• Provide case management services for adult and geriatric patients with caseload of 20 patients • Maintain current BLS certification • Complete required modules and training programs pre-start and annually • Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Work 2 weekends every 6 weeks • Utilize Epic charting system for computerized documentation • Float between facilities within the state based on need • Organize and participate in family meetings • Participate in the development of treatment plans • Determine medical necessity per evidence-based guidelines and utilize InterQual criteria • Coordinate care including discharge planning and care transitions • Arrange durable medical equipment, skilled nursing facility placements, and ambulance transport independently • Work with HMOs, IPAs, and insurance authorization processes • Conduct needs assessments and prior authorizations • Complete data abstraction and utilize ICD-10 and CPT coding knowledge • Ensure compliance with HIPAA guidelines and regulatory requirements • Collaborate with interdisciplinary team including physical therapy, social services, and other support services • Maintain knowledge of CMS guidelines and Joint Commission standards • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Complete skills checklist and required compliance documentation • Self-report time spent on non-billable training modules • Utilize facility-provided communication devices when required for patient care coordination
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
