Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Portland, OR. Key Responsibilities:
• Provide occupational therapy services in home health setting
• Conduct patient evaluations and assessments using standardized tools
• Develop and implement individualized treatment plans and goals
• Perform OASIS documentation and assessments
• Complete patient reassessments and discharge planning
• Provide patient and family education on treatment plans
• Document patient care using Epic Remote Client and Rover mobile application
• Visit up to 6 patients per day maintaining 30 productivity points standard
• Provide therapeutic interventions for various diagnoses including stroke, orthopedic post-op, Parkinson's, dementia, ALS, and MS
• Conduct home safety assessments and equipment evaluations
• Perform splinting, adaptive equipment training, and mobility assessments
• Maintain current BLS certification
• Complete required pre-start and annual training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Maintain valid state occupational therapy license
• Use personal transportation for patient visits with mileage reimbursement
• Complete required compliance documentation and certifications
• Float between facilities within the state as needed
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
