Position Summary:
VitalHire is seeking a skilled and compassionate LPN - LTC to join our growing clinician community. This high-impact opportunity offers a 4x12 Rotating at a leading LTC in Seward, AK. Key Responsibilities:
• Provide direct patient care in long-term care setting with 1:20 patient ratio
• Maintain current BLS certification throughout assignment
• Complete required pre-start and annual training modules (average 16 hours, non-billable)
• Work variable 4x12 or 3x12 hour shifts (either days or nights based on facility needs)
• Work at least 2 of the 3 major winter holidays (Thanksgiving, Christmas, New Years)
• Utilize ECS charting system for documentation
• Float within facility scope to meet staffing needs
• Maintain valid Alaska state LPN license
• Provide minimum of 2 supervisory references covering 1 year of employment in comparable LTC specialty
• Complete required compliance documentation and certifications
• Perform blood glucose monitoring and testing
• Administer medications via oral, IM, SQ, and subcutaneous routes
• Manage feeding tube care including PEG tube feedings
• Insert and manage Foley catheters for male and female patients
• Provide wound care and respiratory treatments including nebulizers
• Perform patient ambulation, bathing, and personal care
• Monitor vital signs and communicate patient status changes to RN/MD
• Maintain isolation precautions when required
• Care for patients with dementia, Alzheimer's, TBI, and various physical/cognitive impairments
• Operate specialized equipment including lifts, glucose monitors, and EKG machines
• Self-report time spent on training modules and maintain completion records
• Utilize facility-provided communication devices when required
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
