Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Santa Rosa, CA. Key Responsibilities:
• Provide case management services for adult and geriatric patients • Maintain current BLS certification and required certifications • Manage patient caseload of approximately 20 patients • Utilize Epic charting system for computerized documentation • Perform discharge planning and care coordination activities • Organize and participate in family meetings • Participate in the development of patient treatment plans • Arrange durable medical equipment, skilled nursing facilities, and ambulance transport independently • Work with HMOs, IPAs, and insurance companies • Determine medical necessity per evidence-based guidelines • Utilize InterQual criteria for patient assessments • Complete needs assessments and order DME as required • Perform data abstraction and maintain accurate documentation • Ensure compliance with HIPAA guidelines and facility policies • Work weekends as scheduled (two weekends every six weeks) • Commit to working 3 of the 5 major holidays • Float between facilities within the state as required • Complete required pre-start and annual training modules • Provide minimum of 2 supervisory references covering 1 year of employment • Maintain travel experience and work independently in case management role
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
