Experience
- Minimum 2 years acute medical Care Manager experience performing Care Coordination/Discharge Planning required
- Previous travel experience required
License & Certification Required
- Current CA state RN license required (Pending License accepted, but must be licensed upon start date)

- Computerized documentation experience required
- Epic charting system experience required
- Knowledge of CMS guidelines
