Experience
- Minimum 2 years acute care hospital Case Manager experience performing Care Coordination and Discharge Planning required
- Previous travel experience required
License & Certification Required
- California RN license required
- Pendings licenses accepted (must be licensed upon start date)

- Ability to organize and participate in family meetings
- Experience with HMOs and IPAs
- Competent with arranging DME, SNF, and ambulance transport independently
- Proficient in Microsoft applications
- Experience with computerized documentation
- Experience assessing patients
