Experience
- Minimum 2 years acute medical Care Manager experience performing Care Coordination and Discharge Planning required
- Previous travel experience required
License & Certification Required
- CA state RN license required
- Pending license accepted, but must be licensed upon start date

- Experience with CMS: Centers for Medicare and Medicaid Services
- Experience with HIPAA guidelines (Health Insurance Portability and Accountability Act)
- Experience with computerized documentation
- Epic charting system experience required
