Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Social Worker to join our growing clinician community. Key Responsibilities:
• Provide care coordination for neurological and cardiac patients
• Conduct biopsychosocial assessments for patients
• Develop comprehensive plans of care to meet patient and caregiver needs
• Coordinate collaborative care with interdisciplinary team members
• Arrange discharge services including home care and equipment coordination
• Document pertinent information to facilitate effective care coordination
• Participate in care conference activities and team meetings
• Make referrals to rehabilitation and long-term care facilities as appropriate
• Refer patients to other disciplines and community resources
• Maintain documentation according to established plan of care
• Provide crisis intervention services when needed
• Assist with various benefit programs including Medicaid, Medicare, and social services
• Coordinate medical transportation and medication assistance programs
• Handle guardianship and power of attorney matters
• Address suicide, homicide, and self-harm risk assessments
• Perform case management and discharge planning activities
• Complete needs assessments and order durable medical equipment
• Maintain current BLS certification throughout assignment
• Utilize Epic charting system for all documentation
• Complete required pre-start and annual training modules
• Float to other campus locations when census is low
• Work minimum 2 weekend days per month
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
