Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Licensed Social Worker to join our growing clinician community. Key Responsibilities:
• Provide social work services for inpatient/acute care patients with focus on neuro and cardiac conditions
• Conduct biopsychosocial assessments and develop comprehensive plans of care
• Coordinate care services and discharge planning for patients and caregivers
• Document pertinent information using Epic charting system to facilitate care coordination
• Participate in interdisciplinary care conferences and collaborative care activities
• Provide crisis intervention services for patients at risk
• Refer patients to rehabilitation facilities, long-term care facilities, and other appropriate resources
• Coordinate home care services and durable medical equipment following discharge
• Assist patients with benefit programs including Medicaid, Medicare, food stamps, and housing resources
• Handle guardianship, power of attorney, and protective services matters
• Conduct needs assessments and case management services
• Address cultural issues and patient vulnerability or safety risks
• Provide knowledge of drug/alcohol treatment options and advance directives
• Handle suicide/homicide/self-harm assessments and interventions
• Float between campus locations as needed based on census requirements
• Work minimum 2 weekend days per month as required
• Complete required pre-start and annual training modules
• Maintain current BLS and LSWAIC certifications
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
