Position Summary:
VitalHire is seeking a skilled and compassionate OT - Therapy Rehab to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Portland, OR. Key Responsibilities:
• Provide occupational therapy services in home health setting
• Conduct patient visits with up to 6 patients per day maintaining 30 productivity points standard
• Maintain current BCLS certification
• Complete required COVID vaccination including primary series and one booster
• Utilize EPIC Remote Client for Home Health charting and documentation on laptop
• Use Rover phone application for patient care coordination
• Complete OASIS documentation requirements
• Provide own reliable transportation for patient visits
• Travel within 60 mile radius with 60-90 minute total drive time per day
• Work 8-hour day shifts with no weekend requirements
• Complete required modules pre-start and annually (average 16 hours non-billable time)
• Self-report time spent on training modules in Workramp transcripts
• Maintain valid OR state licensure
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Adhere to business casual dress code (scrubs allowed but not required)
• Float between facilities within the state as directed
• Use facility-provided communication devices when required for assignment duties
• Treat patients with diagnoses including dementia, Parkinson's, ALS, MS, ortho post-op, and post-stroke conditions
• Provide care for adult and geriatric patient populations
• Complete assessments, evaluations, discharge planning, and patient/family education
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
