Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Anchorage, AK. Key Responsibilities:
• Provide case management services for patients requiring discharge planning • Maintain current BLS certification and valid AK RN license • Utilize Epic charting system for all documentation and patient management • Provide acute care case management for diverse patient populations including neonates, infants, pediatrics, adolescents, adults, and geriatrics • Work Monday through Friday business hours with weekend coverage as needed to meet department needs • Float within case manager services and to affiliate hospitals to cover gaps in coverage • Coordinate care and discharge planning for patients with various diagnoses • Conduct needs assessments and order durable medical equipment (DME) • Develop and implement plans of care for patients • Ensure compliance with CMS (Centers for Medicare and Medicaid Services) guidelines • Maintain strict adherence to HIPAA guidelines for patient privacy and confidentiality • Collaborate with interdisciplinary support teams including IV teams, physical therapy, respiratory services, social services, and pharmacy • Utilize Milliman Guidelines for case management decisions • Work with Microsoft Office/Teams for documentation and communication • Complete required pre-start and annual training modules • Work 2 out of 3 major holidays (Thanksgiving, Christmas, and New Year) • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Self-report time spent on non-billable training modules • Use facility-provided communication devices when required for patient care duties
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
