Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Anchorage, AK. Key Responsibilities:
• Provide case management services for patients requiring discharge planning • Maintain current BLS certification and valid AK RN license • Utilize Epic charting system for patient documentation and care coordination • Complete required modules and training programs pre-start and annually • Work within acute care hospital setting with various patient age groups including neonates, infants, pediatrics, adolescents, adults, and geriatrics • Conduct needs assessments and order durable medical equipment (DME) • Develop and implement plans of care for patients • Coordinate discharge planning and care transitions • Apply Milliman Guidelines in case management decisions • Ensure compliance with CMS Centers for Medicare and Medicaid Services regulations • Adhere to HIPAA guidelines for patient privacy and data protection • Float within case manager services to cover gaps as needed • Work every other weekend or 4 weekend days in a 4-week schedule • Collaborate with interdisciplinary teams including social services, pharmacy, physical therapy, and other support services • Work 2 out of 3 major holidays (Thanksgiving, Christmas, and New Year) • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Self-report time spent on non-billable training modules • Utilize facility-provided communication devices when required for patient care duties • Complete required compliance documentation and certifications
Job Requirements:
- Active, unencumbered license in AK
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
