Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Assess patients and coordinate their care throughout hospitalization • Develop and participate in discharge planning for patients across all age groups • Conduct utilization review to ensure appropriate level of care • Organize and participate in family meetings for treatment planning • Participate in the development of comprehensive treatment plans • Coordinate with HMOs, IPAs, and other insurance providers • Arrange durable medical equipment (DME), skilled nursing facility placements, and ambulance transport independently • Provide case management services for patients with complex social needs including homelessness, mental health crises, and substance abuse • Handle adoption/foster care coordination and protective services processes • Manage advance directives and POLST documentation • Collaborate with interdisciplinary team including Physical Therapy and Social Services • Utilize Epic charting system for computerized documentation • Maintain caseload of up to 25 patients during weekdays and 45 patients on weekends • Work weekend rotation schedule to meet department needs • Complete required pre-start and annual training modules • Commit to working 3 of 5 major holidays as required • Float between facilities within the state as assigned • Use facility-provided communication devices when required for patient care duties • Maintain current state nursing licensure • Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty setting • Self-report time spent on non-billable training modules through Workramp system
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
