Position Summary:
VitalHire is seeking a skilled and compassionate LPN/LVN - Outpatient to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Kernersville, NC. Key Responsibilities:
• Perform telephone triage for patient calls and inquiries
• Provide outpatient clinic nursing care for urology patients aged 18 and up
• Insert and remove Foley catheters for both male and female patients
• Perform phlebotomy and obtain clean catch urine specimens
• Administer medications via IM, SQ, oral, and subcutaneous routes
• Provide oxygen administration using face masks and nasal cannula
• Communicate patient status changes to RN/MD using SBAR format
• Conduct follow-up phone calls and handle pharmacy refill requests
• Interpret laboratory results and ensure proper specimen labeling
• Perform medication reconciliation and pain assessment management
• Complete patient health questionnaires, histories, and allergy documentation
• Monitor vital signs and utilize bladder scanner equipment
• Assist with cystoscopies including Botox injections in sterile field
• Maintain sterile technique during procedures and catheter placements
• Utilize Epic charting system for all documentation
• Float to affiliate clinic locations with one week notice
• Complete 4-5 hours of non-billable training modules pre-start and annually
• Work Monday through Friday, 8-hour shifts from 08:00 to 17:00
• Maintain current BLS certification
• Provide own stethoscope for patient assessments
• Apply de-escalation protocols when managing difficult situations
• Work with adult and geriatric patient populations up to 100 visits per day
Job Requirements:
- Active, unencumbered license in NC
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
