Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Mission Hills, CA. Key Responsibilities:
• Perform case management duties for acute care hospital patients including adolescents, adults, and geriatrics
• Conduct care coordination and discharge planning for patients
• Assess patients using admission criteria and InterQual guidelines
• Develop and participate in treatment plan development
• Organize and participate in family meetings
• Arrange discharge needs including DME, SNF placement, and ambulance transport independently
• Coordinate care for complex patient populations including those with drug/alcohol abuse, homelessness, mental health issues, and advance directives
• Work with interdisciplinary team including social services and transportation departments
• Manage patient ratios of approximately 20 patients during day shifts
• Work every other weekend and commit to working 3 of 5 major holidays
• Utilize Epic charting system for computerized documentation
• Demonstrate proficiency in Microsoft applications
• Work with various insurance types including HMOs and IPAs
• Float between facilities within the state as required
• Complete 40-hour orientation program
• Complete required pre-start and annual training modules
• Provide minimum of 2 supervisory references covering 1 year of employment in comparable specialty
• Maintain current nursing licensure and required certifications
• Use facility-provided communication devices when required for assignment duties
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
