Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Social Worker - Case Management to join our growing clinician community. Key Responsibilities:
• Work as discharge planner for medical/surgical telemetry, ICU, emergency department, progressive care unit, joint/spine, and observation short stay units
• Provide discharge planning services Monday through Friday 8:00 AM to 4:30 PM with occasional weekends
• Focus exclusively on discharge planning rather than behavioral health services
• Maintain current state licensure and complete license verification
• Complete required health screenings including fit test and drug screening
• Maintain current COVID-19, MMR, Hepatitis B, and Varicella vaccinations
• Complete tuberculosis screening via 2-step PPD or IGRA testing
• Complete physical/health statement and maintain health requirements
• Complete mandatory compliance forms and conduct agreements
• Undergo background check and complete security screening processes
• Complete education verification and skills checklist requirements
• Participate in Joint Commission testing and mandatory training programs
• Adhere to patient confidentiality and system security agreements
• Complete conflict of interest disclosure and integrity compliance forms
Job Requirements:
- Active, unencumbered license in MD
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
