Position Summary:
VitalHire is seeking a skilled and compassionate Social Worker - Social Worker to join our growing clinician community. Key Responsibilities:
• Work as discharge planner for medical/surgical telemetry, ICU, ED, PCU, joint/spine, observation short stay, and other units
• Provide discharge planning services Monday through Friday 8:00 AM to 4:30 PM with occasional weekends
• Focus exclusively on discharge planning rather than behavioral health services
• Maintain valid state licensure and complete license verification requirements
• Complete required health screenings including fit testing, drug screening, and vaccinations
• Comply with all facility forms and documentation requirements including code of conduct and confidentiality agreements
• Complete skills checklist and competency assessments
• Maintain current experience in acute hospital setting within the last 2 years
• Participate in Joint Commission testing and compliance activities
• Complete required background checks and verification processes
• Adhere to facility policies and procedures for patient discharge planning
Job Requirements:
- Active, unencumbered license in MD
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
