Position Summary:

Key Responsibilities:
• Work in care management department Monday through Friday 0800-1630 and every other weekend
• Perform hospital discharge planning for adult, pediatric and maternal health patients
• Complete admission assessments for patients
• Create comprehensive discharge plans involving home care, subacute and acute rehab patients
• Work with homeless patients and manage difficult complex discharges
• Work primarily in one unit or rotate between various units as assigned
• Attend daily discharge planning meetings
• Collaborate with other staff members in creating effective discharge plans
• Communicate frequently with families and patients regarding discharge planning
• Complete required compliance documentation and certifications
• Maintain current professional licensure and certifications
• Complete mandatory health screenings and vaccinations
• Adhere to organizational policies and procedures
Job Requirements:
- Active, unencumbered license in MD
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
