Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. Key Responsibilities:
• Provide skilled physical therapy services in home health setting
• Conduct patient visits with 4-5 patients per day achieving 27-30 productivity points
• Perform OASIS documentation and Start of Care (SOC) assessments
• Maintain timely communication and documentation requirements
• Utilize charting systems and documentation tools provided
• Travel to patient homes within assigned territory with occasional radius up to 100 miles
• Provide physical therapy for various conditions including post-operative care, spinal conditions, joint replacements, and neurological disorders
• Develop and implement home exercise programs for patients
• Provide patient and family education on treatment plans and care
• Maintain current BLS certification
• Maintain valid state physical therapy license
• Complete required compliance documentation and training modules
• Participate in on-call rotation as required
• Use personal transportation to reach patient locations
• Complete Hospice/Home Health Checklist as mandatory requirement
• Submit requested time off (RTO) at time of application
• Return all provided documentation tools and equipment
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
