Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Torrance, CA. Key Responsibilities:
• Provide case management services for up to 25 patients with various age groups including neonates, infants, pediatrics, adolescents, adults, and geriatrics
• Perform care coordination and discharge planning functions
• Conduct biopsychosocial assessments and intake screenings
• Develop and participate in treatment plans with multidisciplinary teams
• Coordinate services for home care and equipment following discharge
• Facilitate discharge and contingency planning
• Organize and participate in family meetings and care conferences
• Provide patient and family education, support, and advocacy
• Document care delivery, plans, goals, and patient responses according to plan of care
• Utilize Epic charting system for computerized documentation
• Work with patients experiencing mental health crises, homelessness, and substance abuse issues
• Handle prior authorizations, appeals, and denials processes
• Coordinate with internal and external team members including IV teams, physical therapy, respiratory services, and social services
• Refer patients to appropriate disciplines, resources, rehabilitation, and long-term care facilities
• Manage cases involving guardianship, power of attorney, and advance directives
• Work rotating weekend coverage and manage up to 2 units on weekends
• Complete required modules and training programs pre-start and annually
• Float between facilities within the state as required
• Work 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Maintain current certifications and provide minimum of 2 supervisory references
• Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
