Position Summary:

Key Responsibilities:
• Perform care coordination assessments for initial assessment of patients within 24 hours of admission
• Conduct assessments for readmission and transition planning
• Work collaboratively with social workers and other disciplines to ensure safe, appropriate, and timely transition to next level of care
• Assess patient/family needs to reduce barriers and formulate discharge plans
• Identify unsigned level of care orders and communicate with utilization management nurse to obtain orders from providers
• Review current DRG/LOS within electronic systems to assess discharge planning needs with providers
• Assess risk of readmission for specified patient populations and initiate assigned interventions
• Perform discharge planning coordination by making appropriate referrals to social services, ancillary departments, outpatient case management, DME, and post-acute placement
• Act as liaison by collaborating and communicating daily with physicians, patients, families, nursing, and healthcare team members
• Actively participate in clinical case review/rounds with interdisciplinary team
• Document in electronic medical record including assessments, plans, interventions, barriers, and reassessments
• Manage anticipated discharge dates and ensure pertinent information transfer to post-acute agencies
• Identify barriers early in patient stay and formulate plans with patients, families, and healthcare team
• Identify and report avoidable day variances and service delays from established plan of care
• Represent integrated care management department on various teams and performance outcomes committees
• Ensure patients have follow-up appointments with PCP scheduled prior to discharge
• Maintain effective operations by following policies and procedures
• Maintain current BLS certification
• Perform other related duties as required and directed
Job Requirements:
- Active, unencumbered license in MI
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
