Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Livonia, MI. Key Responsibilities:
• Provide physical therapy services to patients in home health settings
• Maintain current state physical therapy licensure
• Work full-time 40 hours per week serving the Livingston County area
• Conduct occasional weekend visits as needed
• Utilize EPIC system for patient documentation and access
• Complete required onboarding documentation and compliance forms
• Maintain current immunizations including COVID-19, MMR, Hepatitis B, Varicella, and Tdap
• Complete tuberculosis screening through 2 Step PPD or IGRA testing
• Pass required drug screening and background checks
• Complete specialty-specific competency testing and skills checklists
• Participate in Joint Commission testing and education verification
• Maintain professional liability insurance coverage
• Complete fit testing and color vision screening
• Adhere to confidentiality agreements and system security protocols
• Complete physical/health statement and maintain fitness for duty
• Provide valid driver's license and complete fingerprinting requirements
Job Requirements:
- Active, unencumbered license in MI
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems [Epic, Cerner, etc. preferred]
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
