Experience
- Minimum 2 years Acute Care Case Management/Discharge Planning experience required
- Must have hospital experience; Behavioral health experience does not apply
- Previous travel experience required
License & Certification Required
- CA state RN license required (Pending License accepted, but must be licensed upon start date)
Skills Required
- Experience with assessing patients
- Care coordination
- Discharge planning
- Utilization review
- Competent with arranging DME, SNF, and ambulance transport
- Knowledge of CMS, Department of Health, The Joint Commission/Core Measure/National Safety Goals
- Proficient in computerized documentation
- Experience with admission criteria and benefits eligibility
- Ability to organize and participate in family meetings
- Participate in the development of the treatment plan
- Experience with HMOs, IPAs, etc.
- Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, and POLST
Desired Skills
- Experience with diverse patient age groups: Neonates, Adolescents, Adults, Geriatrics
- Experience in surgical services
- Experience with prior authorizations
- Experience within emergency departments, ICU, MS, and needs assessment/order DME
