Experience
- Minimum 2 years Acute Care Case Management/Discharge Planning experience required
- Must have hospital experience, behavioral health experience does not apply
- Previous travel experience required
License & Certification Required
- California state RN license required (pending license accepted but must be licensed upon start date)
Skills Required
- Experience with Assessing patients
- Care Coordination

- Utilization Review
- Competent with arranging DME, SNF, and ambulance transport
- Knowledge of CMS: Centers for Medicare and Medicaid Services regulations
- Knowledge of The Joint Commission/Core Measure/National Safety Goals
- Familiarity with admission criteria and benefits eligibility
- Experience working with patients experiencing homelessness and those with mental health history or in crisis
- Ability to organize and participate in family meetings
- Experience with HMOs, IPAs, etc.

- Experience with computerized documentation
