Experience
- Minimum 1 year of skilled home health experience required
- Experience with OASIS documentation required
License & Certification Required
- Active PT license in Oregon and Washington required
- BCLS certification required

- Ability to lift and transfer patients
- Order supplies
- Transport patients to/from treatment
- Schedule appointments
- Make telephone calls
- Experience with EPIC for charting required
- Experience with home health care procedures required
