Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Rochester, NH. Key Responsibilities:
• Provide direct patient care and assistance with activities of daily living
• Assist patients with personal hygiene, bathing, grooming, and dressing
• Help patients with mobility, transfers, and positioning
• Monitor and record vital signs as directed by nursing staff
• Assist with feeding and ensure proper nutrition and hydration
• Maintain patient safety and comfort at all times
• Document patient care activities and observations accurately
• Communicate effectively with patients, families, and healthcare team members
• Follow infection control protocols and maintain a clean environment
• Assist with medical procedures as directed by licensed nurses
• Respond to patient call lights and requests promptly
• Maintain current CNA certification and complete required continuing education
• Adhere to facility policies, procedures, and safety standards
• Work collaboratively as part of the healthcare team
• Maintain patient confidentiality and HIPAA compliance
Job Requirements:
- Active, unencumbered license in NH
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
