Position Summary:

Key Responsibilities:
• Provide physical therapy services as a Physical Therapist Assistant in a skilled nursing facility setting
• Implement treatment plans under the supervision of a licensed Physical Therapist
• Assist patients with therapeutic exercises and mobility training
• Document patient progress and treatment sessions accurately
• Monitor patient responses to treatment interventions
• Maintain professional communication with patients, families, and healthcare team members
• Follow established protocols for infection control and safety procedures
• Participate in patient care conferences and treatment planning meetings
• Ensure proper use and maintenance of therapeutic equipment
• Maintain current state licensure and required certifications
• Complete continuing education requirements as mandated
• Adhere to facility policies and procedures
• Work scheduled shifts from 08:00 to 16:30
Job Requirements:
- Active, unencumbered license in VA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
