Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading LTC in Medford, OR. Key Responsibilities:
• Provide skilled home health nursing services to adult patients
• Conduct admission assessments for new patients
• Perform start of care visits and documentation
• Complete routine home health visits with 1:1 patient ratios
• Visit up to 6 patients per day maintaining 30 productivity points standard
• Conduct recertification visits and assessments
• Manage patient discharges including safety-related or non-compliance discharges
• Provide resumption of care services when appropriate
• Perform comprehensive wound care and assessment
• Manage medication administration and assessment including intradermal, intramuscular, intravenous infusions, and subcutaneous routes
• Conduct pain assessment and management using verbal and nonverbal pain scales
• Manage PCA pump operations and monitoring
• Implement infection control precautions according to protocols
• Complete OASIS documentation and assessments
• Utilize EPIC charting system for all documentation
• Maintain current BLS certification
• Provide case management services as needed
• Complete required pre-start and annual training modules
• Maintain reliable transportation for patient visits
• Travel within occasional maximum radius of 50+ miles for patient visits
• Float between facilities within the state as required
• Maintain current COVID vaccination including primary series and one booster
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Self-report time spent on non-billable training modules
• Complete computerized documentation requirements
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
