Position Summary:
VitalHire is seeking a skilled and compassionate RN - Hospice to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Charlotte, NC. Key Responsibilities:
• Provide crisis response nursing care for hospice patients with 6 months or less to live
• Complete an average of 5 patient visits per day in outpatient hospice setting
• Assess and manage patient symptoms including altered mental status, anorexia, cachexia, constipation, diarrhea, and nausea/vomiting
• Perform pain assessment and implement pharmacologic and non-pharmacologic pain interventions
• Administer medications using WHO Analgesic Ladder principles and breakthrough dosing protocols
• Conduct intake assessments and interviews with patients and families
• Provide patient and family education and support regarding end-of-life care
• Set goals collaboratively with patients and families
• Perform pressure ulcer staging and implement positioning strategies to prevent shearing and pressure ulcers
• Utilize various pain scales for verbal and nonverbal patients
• Ensure proper disposal of medications according to protocols
• Document patient care using Epic Remote Client charting system
• Participate in on-call rotation for weekend coverage
• Work 8-hour shifts from 08:00 to 17:00
• Complete 40 hours of unit orientation in addition to hospital orientation
• Complete required non-billable training modules pre-start and annually
• Maintain current BLS certification
• Comply with Medicare state regulations and documentation requirements
• Provide culturally aware care to primarily geriatric patient population
• Work 2 out of 3 major holidays as required
• Adhere to fatigue policy limiting work to 5 consecutive days and 60 hours per week
• Complete crisis response compliance testing within 48 hours of assignment
• Maintain valid nursing license and provide supervisory references
Job Requirements:
- Active, unencumbered license in NC
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
