Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Rehab in Portland, OR. Key Responsibilities:
• Provide skilled nursing care in home health settings for adult and geriatric patients
• Conduct patient visits with up to 6 patients per day maintaining 30 productivity points standard
• Perform admission assessments and start of care procedures
• Complete discharge planning and discharge due to safety or non-compliance
• Conduct recertification and resumption of care visits
• Provide routine nursing visits and wound care management
• Perform medication assessment and management
• Conduct pain assessment and management utilizing appropriate pain scales
• Start and maintain IV access and administer intravenous infusions
• Perform phlebotomy procedures
• Administer medications via intramuscular and subcutaneous routes
• Implement infection control precautions
• Manage PICC lines, G-tubes, ostomy care and education
• Provide catheter care and education
• Utilize wound vacs and provide disease management
• Document patient care using EPIC Remote Client for Home Health and Rover phone application
• Complete Oasis documentation requirements
• Maintain current BLS certification
• Hold valid nursing licensure in both Oregon and Washington states
• Complete required pre-start and annual training modules
• Self-report time spent on non-billable training modules
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty
• Maintain own reliable transportation for patient visits
• Float between facilities within the state as required
• Utilize facility-provided communication devices when supplied
• Comply with business casual dress code requirements
Job Requirements:
- Active, unencumbered license in OR
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
