Position Summary:
VitalHire is seeking a skilled and compassionate Social Services - MSW (Masters of Social Work) to join our growing clinician community. Key Responsibilities:
• Provide social work services to participants as a member of the interdisciplinary team • Spend majority of time in direct service activities including one-on-one and group counseling • Interact with team members and provide ongoing assistance to participants for social issues • Conduct comprehensive social work assessments and interventions • Provide perspective on social, emotional, cognitive and ethnic factors impacting individualized care plans • Access community and financial resources for participants • Ensure continuity of care across the continuum • Conduct assessments in participant homes and other care settings as needed • Float between multiple Adult Day Care Program locations • Maintain current BLS certification and required state social work licensure • Complete required modules and training programs pre-start and annually • Utilize Epic charting system for documentation • Use facility-provided communication devices and expense tracking systems • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting • Self-report time spent on non-billable training modules • Travel between work locations during scheduled work hours for participant visits
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
