Position Summary:
VitalHire is seeking a skilled and compassionate Social Services - MSW (Masters of Social Work) to join our growing clinician community. Key Responsibilities:
• Provide social work services to participants as a member of the interdisciplinary team • Conduct direct service activities including one-on-one and group counseling • Perform comprehensive social work assessments and interventions • Provide perspective on social, emotional, cognitive and ethnic factors for individualized care plans • Access community and financial resources for participants • Ensure continuity of care across the continuum • Conduct home-based assessments and interventions for participants • Maintain ongoing assistance to participants to address social issues • Interact with interdisciplinary team members for coordinated care • Document services using Epic charting system • Complete required modules and training programs pre-start and annually • Maintain current BLS certification • Maintain valid Washington state social work licensure (LICSW, LSWAIC, or LASW) • Utilize facility-provided communication devices including laptop and cell phone • Float between facilities within the state as required • Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty • Self-report time spent on non-billable training modules • Travel between work locations for participant home visits during business hours • Work with geriatric population in adult day care program setting
Job Requirements:
- Active, unencumbered license in WA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
