Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Apple Valley, CA. Key Responsibilities:
• Assess patients and perform utilization review • Coordinate care and develop treatment plans • Conduct discharge planning and arrange post-acute care services • Organize and participate in family meetings • Work with HMOs, IPAs, and insurance authorization processes • Arrange durable medical equipment (DME), skilled nursing facility placement, and ambulance transport independently • Manage caseload of up to 25 patients during weekdays and up to 45 patients on weekends • Work minimum of 4 weekend shifts in a 6-week period • Commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Utilize Epic charting system for computerized documentation • Collaborate with interdisciplinary team including Physical Therapy and Social Services • Provide care across all age groups including neonates, adolescents, adults, and geriatrics • Address complex patient situations including adoption/foster care, substance abuse treatment, homelessness, mental health crises, advance directives, and protective services • Float between facilities within the state as required • Complete required pre-start and annual training modules • Maintain minimum of 2 supervisory references covering 1 year of employment in comparable specialty • Use facility-provided communication devices when required for patient care duties • Self-report time spent on non-billable training modules
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
