Position Summary:
VitalHire is seeking a skilled and compassionate PT - Physical Therapist to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Hesperia, CA. Key Responsibilities:
• Provide physical therapy services in home health setting
• Conduct up to 6 patient visits per day maintaining 30 productivity points standard
• Maintain current BCLS certification
• Utilize EPIC charting system and Oasis documentation
• Provide patient and family education on treatment plans
• Develop and implement home exercise programs
• Treat various conditions including post-operative care, neurological disorders, orthopedic conditions, and stroke rehabilitation
• Work with adult and geriatric patient populations
• Complete required modules and training programs pre-start and annually
• Maintain valid state physical therapy licensure
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Work one weekend shift per contract
• Maintain reliable transportation for patient visits
• Self-report time spent on non-billable training modules
• Complete required compliance documentation and certifications
• Follow interdisciplinary care plans with physical therapy, interpretation services, and social services teams
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
