Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Santa Rosa, CA. Key Responsibilities:
• Provide case management services for adult and geriatric patients
• Maintain current BLS certification
• Manage patient caseload of up to 20 patients
• Utilize Epic charting system for computerized documentation
• Participate in discharge planning and coordinate care transitions
• Organize and participate in family meetings
• Participate in the development of treatment plans
• Arrange DME, SNF and ambulance transport independently
• Work with HMOs, IPAs and other insurance providers
• Determine medical necessity per evidence-based guidelines
• Utilize InterQual criteria for patient assessments
• Conduct needs assessments and order durable medical equipment
• Process prior authorizations and benefits eligibility verification
• Collaborate with interdisciplinary team including IV teams, physical therapy, respiratory services, social services, and other support staff
• Maintain compliance with HIPAA, OSHA, and Joint Commission requirements
• Complete required modules and training programs pre-start and annually
• Work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
• Float between facilities within the state as needed
• Provide minimum of 2 supervisory references covering 1 year of employment within comparable specialty/setting
• Complete 40 hours of orientation
• Maintain current work history within 2 years in case management
• Self-report time spent on non-billable training modules
• Utilize facility-provided communication devices when required
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
