Experience
- Minimum 2 years acute medical Care Manager experience performing Care Coordination required
- Previous travel experience required
License & Certification Required
- Active RN license in California required; pending license accepted if licensed upon start date
Skills Required
- Experience with Assessing patients
- Care Coordination
- Discharge Planning
- Competent with arranging DME, SNF, and ambulance transport
- Charting experience with Epic required
Desired Skills
- Trauma Level I experience preferred
- Trauma Level II experience preferred
- Knowledge of adoption/foster care and drug/alcohol abuse treatment options
- Experience with patients experiencing homelessness or with mental health history
- Familiarity with CMS, Department of Health, and HIPAA guidelines
