Experience
- Minimum 2 years acute medical Care Manager experience performing Care Coordination required
- Previous travel experience required
License & Certification Required
- Active RN license in California required
- PGA (Pending License accepted) must be licensed upon start date
Skills Required
- Experience with care coordination
- Competent with arranging DME, SNF, and ambulance transport
- Experience with computerized documentation
- Knowledge of HIPAA guidelines
- Ability to organize and participate in family meetings
- Experience with assessing patients
- Experience with discharge planning
- Knowledge of CMS and Department of Health requirements
- Understanding of appeals and denials
- Familiarity with DRG (Diagnosis Related Groups)
- Understanding of worker's compensation case management
- Epic charting system experience required
Desired Skills
- Experience with trauma level I and II facilities preferred
- Knowledge of patients experiencing homelessness or with mental health history preferred
- Experience with adoption/foster care and drug/alcohol abuse treatment options preferred
