Experience
- Minimum 2 years Acute Care Case Management/Discharge Planning experience required
- Previous travel experience required
- Hospital experience required; Behavioral health experience does not apply
- Experience with assessing patients, care coordination, discharge planning, and utilization review
- Experience with HMOs, IPAs, and patients experiencing homelessness or with mental health history
- Competent with arranging DME, SNF, and ambulance transport
License & Certification Required
- CA state license required; pending license accepted but must be licensed upon start date
- Registered Nurse (RN) certification required
Skills Required
- Knowledge of admission criteria and benefits eligibility
- Experience with CMS (Centers for Medicare and Medicaid Services) guidelines
- Proficient in computerized documentation, specifically Epic EMR
- Knowledge of Discharge Planning and disease management
- Competent in prior authorizations and surgical services
- Familiarity with The Joint Commission/Core Measure/National Safety Goals
