Experience
- Minimum 2 years Acute Care Case Management/Discharge Planning experience required
- Previous travel experience required
- Must have hospital experience; behavioral health experience does not apply
License & Certification Required
- California state nursing license required; pending licenses accepted if licensed by start date

- Experience with assessing patients
- Care coordination skills
- Discharge planning skills
- Utilization review skills
- Competent with arranging durable medical equipment (DME), skilled nursing facility (SNF) placements, and ambulance transport
- Knowledge of adoption/foster care processes, drug/alcohol abuse treatment options, mental health issues, and patients experiencing homelessness
Desired Skills
- Experience with HMOs and IPAs
- Ability to organize and participate in family meetings
- Experience with computerized documentation, specifically Epic
