Experience
- Minimum 2 years Acute Care Case Management experience required
- Hospital experience required; Behavioral health experience does not apply
- Previous travel experience required
- Experience with assessing patients, care coordination, discharge planning, and utilization review
- Experience with HMOs, IPAs, and knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, and patients with mental health history or in crisis
License & Certification Required
- Current CA state RN license required; pending license accepted if licensed upon start date
Skills Required
- Competent with arranging DME, SNF, and ambulance transport
- Knowledge of CMS (Centers for Medicare and Medicaid Services) and department of health regulations
- Proficient in discharge planning and disease management
- Experience with computerized documentation, specifically Epic
