Compensation
- Gross Weekly Pay: $2772.48
- Weekly Taxable Amount: $1000.0
- Weekly Stipend/Non Taxable Amount: $1772.48
You must be duplicating expenses to receive stipends. If you are driving versus relocating, we will instead offer mileage reimbursements. Rates assume Travel. Locals within 50 miles may have a rate adjustment.

Experience
- Minimum 2 years acute medical Care Manager experience performing Care Coordination required
- Previous travel experience required
- Experience with assessing patients and care coordination
- Experience with discharge planning required
- Experience with HMOs, IPAs, and related systems preferred
- Experience working with adult and geriatric patient populations required
License & Certification Required
- California state license required; pending license accepted if licensed upon start date
Skills Required
- Proficiency in using Epic charting system required
- Knowledge of HIPAA guidelines required
- Ability to independently arrange DME, SNF, and ambulance transport required
- Competency in discharge planning required
- Experience with care coordination required
- Knowledge of advance directives and protective services process required
Desired Skills
- Trauma Level I and II experience preferred
- Knowledge of drug/alcohol abuse treatment options preferred
- Experience with patients experiencing homelessness preferred
- Experience with mental health history or in crisis preferred
Requirements
- Current Resume with up to 7 year work history, with gaps explained - position, unit, job type, city, state must be indentified for each assignment
- Negative TB Test (Within 12 Months)
- Titers - MMR/Hep B/Varicella
- Current - Tdap/Flu Vaccinations
- Must be able to pass pre-employment, background and drug screening
