Position Summary:
VitalHire is seeking a skilled and compassionate SLP - SLP to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading Hospital in Cottonwood, AZ. Key Responsibilities:
• Provide speech language pathology services for pediatric outpatient (85%) and adult outpatient (15%) populations
• Conduct adult acute care services one day per week
• Maintain current BLS certification
• Hold and maintain CCC-SLP certification
• Manage daily patient load of 12-14 patients
• Work Tuesday through Saturday schedule (5x8 hours, 8:00-4:30)
• Provide outpatient services Tuesday through Friday
• Cover acute care services on Saturdays
• Work 2 out of 3 major holidays as required
• Utilize Cerner charting system for documentation
• Float between units within hospital or to affiliate hospitals as needed
• Complete required compliance testing within 72 hours after signing
• Complete all required modules during first week of orientation (average 6 hours)
• Provide two professional references prior to start date
• Complete required background checks and state releases as applicable
• Maintain AZ Fingerprint Clearance Card
• Adhere to business casual dress code requirements
• Complete required training programs and certifications annually
• Provide services across multiple age groups including adolescents, adults, pediatrics, and geriatrics
Job Requirements:
- Active, unencumbered license in AZ
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
