Position Summary:
VitalHire is seeking a skilled and compassionate RN - Home Health to join our growing clinician community. This high-impact opportunity offers a 5x8 days at a leading LTC in Bay City, MI. Key Responsibilities:
• Implement full time professional nursing service to patients in their homes under clinical supervision • Initiate and provide medically prescribed skilled nursing care and health counseling • Contribute to treatment of medical conditions and achievement of rehabilitation potential • Reduce pain and other symptoms through skilled nursing interventions • Make initial visits to assess patient's physical and emotional status, needs, and resources • Conduct patient assessments through observation, interview and analysis of records • Prepare nursing plan of care in accordance with physician's plan of treatment • Provide patient teaching using Care Plan/Standard of Care • Complete infusion and wound care procedures • Utilize OASIS documentation system • Submit documentation on time and maintain accurate hour tracking • Maintain current BLS (AHA) certification • Maintain valid Michigan state nursing license • Work 8-hour day shifts with 40-hour guarantee
Job Requirements:
- Active, unencumbered license in MI
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
