Position Summary:
VitalHire is seeking a skilled and compassionate CNA - SNF to join our growing clinician community. This high-impact opportunity offers a 5x8 Days at a leading SNF in Pennsburg, PA. Key Responsibilities:
• Provide direct patient care in skilled nursing facility setting
• Assist patients with activities of daily living including bathing, dressing, and mobility
• Monitor and record vital signs and patient observations
• Assist with feeding and ensure proper nutrition and hydration
• Maintain patient safety and comfort at all times
• Document patient care activities and changes in condition
• Communicate effectively with nursing staff and healthcare team members
• Follow infection control protocols and maintain clean environment
• Assist with transfers and positioning of patients
• Respond to patient call lights and requests promptly
• Support patients with personal hygiene and grooming needs
• Maintain current CNA certification and required training
• Adhere to facility policies, procedures, and regulatory requirements
• Work collaboratively as part of interdisciplinary care team
Job Requirements:
- Active, unencumbered license in PA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
