Position Summary:
VitalHire is seeking a skilled and compassionate RN - Case Management to join our growing clinician community. Key Responsibilities:
• Perform acute medical care coordination and case management services with 2+ years of experience • Maintain patient ratios of 1:25 in acute care setting • Float to affiliate facilities as required • Work weekend rotation shifts as scheduled • Complete 40-hour orientation program upon assignment start • Utilize Epic charting system for computerized documentation • Assess patients and coordinate comprehensive care plans • Participate in discharge planning and arrange post-acute care services • Organize and participate in family meetings and treatment plan development • Coordinate with HMOs, IPAs, and various insurance providers • Arrange durable medical equipment (DME), skilled nursing facility placements, and ambulance transport independently • Demonstrate knowledge of adoption/foster care, substance abuse treatment, homelessness resources, mental health services, advance directives, POLST, and protective services • Work with adult and geriatric patient populations up to 25 census daily • Complete required pre-start and annual training modules • Commit to working 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Maintain valid state RN licensure and required certifications • Provide minimum of 2 supervisory references covering 1 year of comparable specialty experience • Utilize facility-provided communication devices when required for assignment duties • Self-report time spent on non-billable training modules through Workramp system • Comply with facility dress code requirements (turquoise scrubs or business casual)
Job Requirements:
- Active, unencumbered license in CA
- Minimum of 1-7 years of relevant experience, depending on specialty
- Current Certifications: BLS and/or ACLS, plus any specialty-specific requirements
- Experience with EMR systems (Epic, Cerner, as applicable)
Other Required Documentation:
- Current Resume listing detailed work history (MM/YYYY format)
- Two current clinical references
- State-required professional license
- Physicial exam (completed within the past 12 months)
- TB skin test or Quantiferon Gold (within the past 12 months)
- Immunity Titers - MMR/Hep B/Varicella (declinations may be accepted)
- Current Tdap and Flu vaccinations
- Must pass background check and drug screening
